An Alternative for Traditional Nursing Homes
Kina’ole Case Management’s senior program is for Medicaid recipients that helps pay for care outside of the nursing home environment. Private pay individuals are also accepted. Program participants live in foster family homes that care for up to three persons.
This program provides a family-like environment for the aging, rather than an institutional nursing home.
1. Eligibility Guidelines
For Hawaii residents to receive financial assistance from the government, there are certain eligibility requirements they must meet:
- Receiving Supplemental Security Income, abbreviated as SSI (if eligible).
- If not receiving Long-Term Care Medicaid, one must apply for assistance.
- The applicant must need the level of care typically provided in a nursing home and validated by a physician. A physical examination by a doctor in the month preceding acceptance into the program or within seven days following acceptance.
- The applicant must be placed in care by a Case Management Agency and continue to receive case management services by the agency.
- Lastly, the care recipient cannot be related to the foster family.
2. Benefits and Services
Hawaii residents who are aged 65 and older and have not applied for long-term care Medicaid can do so online, in person at a benefits office, or by calling 877-628-5076.
To apply for the program, you must go through a Case Management Agency (CMA), either by contacting them directly or going through a referral (CMA list). CMAs can also help applicants apply for Long-Term Care Medicaid.
Please Note: Each agency is independently contracted with various Medicaid Health Plans of their choosing. It’s important to make sure that both are in the provider network for one’s health plan.
3. How to Apply
