We are a registered elderly foster care placement agency and offer onsite monthly visits when needed.

What Is a Foster Home?
A foster home is a private home in the community where a caregiver provides 24-hour care to residents who need a foster home level of care.
For many families, a foster home offers a quieter, home setting for their loved one. Kina’ole Case Management helps you find a home and stays involved with monthly nurse visits after placement.
How Foster Home Placement Works
Placement starts with a free consultation. We learn about your loved one’s health and care needs.
Next, our team completes an initial assessment. The assessment helps us understand which caregiver and home will be the best fit. A licensed Registered Nurse then creates an individualized plan of care.
We match your loved one with a community foster home caregiver who can provide 24-hour care. Same-day admissions are available.
After placement, a Registered Nurse monitors your loved one every month. Our after-hours nurse line is available when questions come up outside office hours.
Medicaid and Private Pay Accepted
We work with families who pay privately and with patients who have Medicaid. Ask us about your coverage during your free consultation. Same-day admissions are available for patients who need placement quickly.
Hospital Discharge and Transitions
Leaving the hospital and moving into a foster home is a big change. We help patients and families plan each step of the move from the hospital to a foster home.
